Advanced Care Planning (ACP) documentation is often scattered across the EHR, making it challenging to find and act upon in times of stress or urgency. UCSD Health’s Quality Incentive Program (QIP) ACP metric captures the number of patients in the QIP population with documented ACP activities. Currently, UCSDH is not meeting our goal in this metric, and therefore are not receiving state funding for this metric via the QIP program. Our aim is to increase our current ACP completion rate (47.89%) to 70.32% by 12/31/2025.
Our AI-driven approach is to improve the visibility and immediacy of ACP conversations and documents by creating concise, structured summaries of existing ACP information stored across the EHR. These structured summaries are available at a glance at the main page of the EHR, and contain hyperlinks to full documents as necessary. Our pilot aims to automate providing clinicians with workable summaries of patient ACP wishes in an accessible format, and help them quickly navigate to ACP documentation from anywhere within the EHR.
